Pharmacodynamics
Cardonate is a combination drug, the action of which is due to the synergistic effects of its constituents. L-carnitine is a trimethylammonium (betaine) derivative of g-amino-b-hydroxybutyric acid, refers to agents with anabolic action. It is synthesized in the liver, kidneys and central nervous system from the amino acids lysine and methionine with the participation of iron and ascorbic acid, in blood plasma it is present in free form and in the form of acylcarnitine esters. Carnitine is the main cofactor and regulator of fatty acid metabolism in the myocardium, liver and skeletal muscles, the only carrier of long-chain fatty acids to mitochondria, where their beta-oxidation to acetyl-CoA occurs, which is a substrate for the formation of ATP in the Krebs cycle.
It promotes the release of metabolites and toxic substances from the cytoplasm, improves metabolic processes. Under normal oxygen supply conditions, it switches cell metabolism to oxidation of fatty acids as the most energy-intensive substrate, and under hypoxic conditions, L-carnitine removes toxic metabolites of fatty acids from the mitochondria, switching cell metabolism to oxidation of glucose, thus implementing an antihypoxic effect. It increases performance, accelerates growth, leads to an increase in muscle mass and a decrease in the mass of adipose tissue due to the lipolytic effect, reduces the peripheral action of thyroid hormones, and helps to normalize the basal metabolism in hyperthyroidism. Reduces symptoms of physical and mental fatigue, has a neuro-, hepato- and cardioprotective effect, exhibits an anti-ischemic effect, helps limit the infarction zone, improves the processes of nerve impulse transmission in synapses and axons by increasing the synthesis of choline and acetylcholine, glutamic acid, reduces the concentration of ammonia in the nervous tissue, stimulates cellular immunity. Eliminates functional disorders of the nervous system in patients with chronic intoxication, including alcohol genesis, softens the course of withdrawal syndrome, inhibits dystrophic changes in the retina. Carnitine exhibits an antiketogenic effect, reducing the production of ketone bodies from higher fatty acids.
Lysine is an essential amino acid, participates in all processes of assimilation and growth, promotes ossification and growth of bone tissue, stimulates cell mitosis, improves oogenesis and spermatogenesis, has a direct antiviral effect on herpes simplex viruses.
Coenzyme of vitamin B12 (cobamamide) has anabolic activity, activates the metabolism of carbohydrates, proteins and lipids, participates in the synthesis of labile methyl groups, in the formation of choline, methionine, nucleic acids, creatine, promotes the accumulation of compounds with sulfhydryl groups in erythrocytes. As a growth factor, it stimulates bone marrow function, which is necessary for normoblastic erythropoiesis. Cobamamide helps to normalize liver and nervous system dysfunctions, has an antiketogenic effect, activates the blood coagulation system, in high doses causes an increase in thromboplastin and prothrombin activity.
Coenzyme of vitamin B1 (cocarboxylase) has a regulating effect on metabolic processes in the body. It plays a particularly important role in carbohydrate and fat metabolism, primarily in the oxidative decarboxylation of keto acids (pyruvic, a-ketoglutaric), as well as in the pentose phosphate pathway of glucose breakdown. Reduces the level of lactic and pyruvic acid in the body, improves glucose absorption, trophism of nervous tissue, and has a cardioprotective effect.
Coenzyme vitamin B6 (pyridoxal-5-phosphate) plays an important role in metabolism, is necessary for the normal functioning of the central and peripheral nervous system. It is a coenzyme of a large number of enzymes that act on the non-oxidative metabolism of amino acids (decarboxylation, transamination processes). Participates in the metabolism of tryptophan, methionine, cysteine, glutamic and other amino acids. It is of great importance for the metabolism of histamine as a coenzyme of histaminase, helps to normalize lipid metabolism, increases the amount of glycogen in the liver, improves its detoxifying properties. Catabolizes neuromuscular processes, which are especially important in childhood with mental and physical retardation, chronic fatigue and asthenia.
Pharmacokinetics
Not studied.
Indications
In adults and children : physical and mental overstrain, including in athletes, decreased performance, asthenic syndrome, vegetative-vascular dystonia, during the recovery period after somatic and infectious diseases, as well as surgical interventions, acetonemic syndromes of various origins, liver diseases (hepatitis, cirrhosis), metabolic cardiopathies, chronic heart failure (as part of complex therapy), to improve ossification in the complex therapy of osteoporosis and bone fractures.
In children: delayed mental and physical development (hypotrophy, anorexia, growth retardation).
In adults: peripheral neuritis, neuralgia, radiculopathy, as part of complex therapy of acute and chronic cerebrovascular accidents (cerebral strokes and chronic cerebral ischemia), as part of complex therapy of ischemic heart disease (angina pectoris, myocardial infarction), hyperhomocysteinemia, as part of complex therapy of alcohol dependence syndrome, including withdrawal, hyperthyroidism.
In obstetrics and gynecology: gestosis of pregnant women, fetoplacental insufficiency and intrauterine growth retardation of the fetus.
Contraindications
Hypersensitivity to the components of the drug, malignant neoplasms, gastric ulcer or duodenal ulcer in the active phase.
Interaction with other medicinal products and other types of interactions
When used simultaneously with levodopa, its therapeutic activity may decrease due to interaction with pyridoxal. Cardonate may enhance the action of antihypertensive agents, cardiac glycosides, peripheral vasodilators, insulin and other hypoglycemic agents.
Features of application
The recommended doses of the drug should not be exceeded.
The ability to influence the reaction speed when driving a vehicle or working with machinery
It doesn't affect.
Use during pregnancy or breastfeeding
The drug should be used by pregnant women only as prescribed and under the supervision of a doctor. It is unknown whether the active ingredients of the drug penetrate into breast milk, so during breastfeeding, Cardonat can be used when the expected benefit to the mother outweighs the potential risk of side effects in the child.
Method of administration and dosage
For adults and children over 15 years of age, take Cardonate 1-2 capsules 3 times daily after meals. For children aged 5-15 years, take 1 capsule 2 times per day. Children aged 1 to 5 years are recommended to take 1 capsule 1 time per day. When drying children with milk, take 1 to 5 capsules, first dissolve them in 50-100 ml of either fruit juice or sweet water.
No dosage adjustment is required for vaginal treatment.
For anorexia, take the drug 15-20 weeks before taking it.
If it is difficult to wrap the capsule (children, patients in the postoperative period, individuals who have suffered a stroke), it is allowed to break it before taking the capsule in juice or sweet water instead.
The average duration of the treatment course is from 3 years to 3 months.
Children
For use by children aged 1 year and over.
Overdose
Tachycardia, dyspeptic disorders, agitation, increased or decreased blood pressure may occur, but they disappear after discontinuation of the drug and do not require treatment.
Adverse reactions
The drug should be well tolerated, but, as a rule, in case of individual sensitivity, allergic reactions may occur, including various sagging on the skin, itching of the skin, kropivyanka, angioneurotic lesion. In such an outburst, treatment must be taken and turned to a doctor.
